Community-Based Services

Avant Supported Living’s Community-Based services assist individuals with developmental disabilities by providing in-home respite, personal assistance, and one-to-one community networking services to help individuals perform activities of daily living more easily. Our goal is to give individuals with disability the tools they need so they may carry out key life tasks as independently as possible while communicating more effectively in the empowered community.

Community Networking

Sometimes, jumping into a big group isn’t the easiest or most comfortable way to connect with the world around you. If you or someone you know thrives with a more personal touch when exploring the community, our Community Networking at Avant Supported Living could be a great fit.

We offer one-on-one support designed for individuals who might find group settings challenging, whether due to medical needs, behavioral preferences, or simply a desire for a quieter approach. Consider having a dedicated guide to your community, someone who understands your rhythm and helps you connect with local opportunities in ways that feel genuinely comfortable and engaging for you.

This support can include:

  • Personalized Community Engagement: One-on-one time focused on your unique interests and goals for getting involved.
  • Transport Assistance: Reliable transportation to appointments, activities you want to participate in, or errands that need to be done.
  • Building Connections: Support in developing social skills and fostering meaningful relationships with others.
  • Finding What’s Available: Assistance in understanding and accessing the different resources our community offers.
  • A Comfortable Pace: Flexible support that respects your need for a calm and less overwhelming environment.
  • Exploring Your Passions: Encouragement and support to discover new hobbies, explore your interests, and try new things.

Our goal is to empower you to connect with your community confidently and in a way that truly reflects who you are and what you want to experience. If this sounds like the kind of support you’re looking for, we’d love to talk more!

In-Home Respite

Life has its moments where extra support is needed at home for a little while. Maybe the person who usually provides assistance has a trip planned, an unexpected situation arises, or they simply need a chance to step back and take a breather. That’s where Avant Supported Living’s in-home respite comes in.

We offer temporary support right in the comfort of home for individuals who need it on a short-term basis. Our aim is to ensure a seamless experience, providing consistent assistance so things continue smoothly in a familiar setting.

This service is about offering reassurance and reliable support when there’s a temporary change in the usual support system. It’s about providing that bridge, ensuring the individual’s well-being and comfort are always the priority, and offering peace of mind during those times.
